Medicaid Case Management in DFW: Asking for a Referral
Updated: 5 days ago
Having health coverage and knowing how to use it are two different things. Many families in Dallas-Fort Worth have Medicaid and still juggle appointments, plan phone numbers, school paperwork and community programs on their own.
What Medicaid is, in one line
According to Texas Health and Human Services, Medicaid and the Children's Health Insurance Program (CHIP) provide health coverage for low-income children, families, seniors and people with disabilities. Plans and benefits are set by the state and your health plan, so confirm details with them.
Where case management fits
Coverage pays for services. Case management helps you line up those services and the community supports around them. That can mean making sense of a care plan, keeping appointments and follow-ups in order, and knowing who to call with which question.
AEIC offers a referral route for Medicaid members through its CPW pathway. Staff review each request and determine eligibility and service fit. A request is not a promise that it will be accepted, and this post does not tell you what your plan covers.
What to have ready
Your name and the best way to reach you
The name of your Medicaid health plan, if you know it (you do not need it to ask)
A short description of the kind of help you want, in your own words
Please leave out detailed medical history. Staff will talk with you about what is needed.
What happens next
You submit the referral request.
AEIC staff review it and determine eligibility.
If it is a fit, staff follow up about next steps. We cannot promise a response time.
Safety note
For a medical emergency, call 911. For emotional distress or thoughts of suicide, call or text 988 to reach the 988 Suicide and Crisis Lifeline, available 24/7. AEIC case management is not a crisis service.
