Texas Medicaid Recipients

Case management services are provided to help Medicaid-eligible persons gain access to necessary medical, social, educational, and other services related to their health condition/health risk or for a pregnant woman, a high-risk condition.

Do you also need advocacy services like a staff member accompanying you and your child to an ARD/IEP meeting or advocacy during  DFPS meetings?

Texas Medicaid Recipients

Mental Health Targeted Case Management (MHTCM) as part of the Texas Medicaid Program are persons, regardless of age, with a diagnosis or diagnoses of mental illness or serious emotional disturbance (SED) and who have been determined via a uniform assessment process to need MHTCM services. *Persons of any age with a single diagnosis of intellectual and developmental disabilities (IDD) and related conditions, or a single diagnosis of substance use disorder (SUD) are not eligible for MHTCM services. MHTCM services are furnished to assist persons in gaining access to needed medical, social, behavioral, educational, and other services and supports.

Mental Health Rehabilitative Services are defined as providing assistance in maintaining or improving functioning and may be considered rehabilitative when necessary to help a person achieve a rehabilitation goal as defined in their plan of care. Mental health rehabilitative services are provided to a person with a serious mental illness (SMI).

Mental health rehabilitative services are age-appropriate, individualized, and designed to ameliorate functional impairments that negatively affect any of the following:

• Community integration

• Community tenure

• Behaviors resulting from SMI or severe emotional disturbance (SED) that interfere with a person’s ability to remain in the community as a fully integrated and functioning member of that community Mental health rehabilitative services may include:

• Medication training and support services

• Psychosocial rehabilitative services

• Skills training and development

• Crisis intervention services

* We also welcome referrals for clients  with mental health concerns related to medical diagnosis (INFERTILITY, HIV/AIDS, CHRONIC RESPIRATORY DISORDER, AND MORE)

Medicare Case Management

AEIC BridgeWell Program

​

BridgeWell is AEIC’s Medicare-supported care coordination program that helps adults manage ongoing chronic and behavioral health conditions such as diabetes, hypertension, depression, anxiety, heart disease, COPD, and arthritis.

Our team of licensed clinicians, Community Health Workers, and Peer Support Specialists work together each month to bridge gaps in care, improve wellness, and connect clients to essential resources. BridgeWell ensures every client receives compassionate support to stay on track with medications, appointments, and health goals—promoting a healthier, more independent life.

BridgeWell is designed for:

  • Adults enrolled in Medicare (traditional plans)

  • Individuals living with two or more chronic or behavioral health conditions

  • Clients recently discharged from a hospital or emergency visit who need follow-up support

  • Individuals who benefit from help managing appointments, medications, or lifestyle changes

  • Anyone who could use extra support connecting to community, nutrition, or transportation resources
